This Notice describes how medical information about you may be used and disclosed, your rights regarding your health information, and our legal responsibilities under the Health Insurance Portability and Accountability Act (HIPAA) and applicable federal confidentiality regulations, including 42 CFR Part 2.

Special Protection for Substance Use Disorder Records
Certain records related to substance use disorder diagnosis, treatment, or referral are protected under federal law, including 42 CFR Part 2.
These records generally may not be used or disclosed without your written consent unless otherwise permitted or required by law. Federal law also protects against unauthorized redisclosure of this information.
You have the right to:
- provide or withhold consent for disclosures,
- revoke consent in writing,
- request restrictions on certain disclosures,
- and receive an accounting of disclosures as permitted by law.

We may use and disclose your health information for purposes including:
- Treatment: To provide, coordinate, or manage your healthcare and treatment services.
- Payment: To obtain payment for services provided to you.
- Healthcare Operations: For administrative, quality improvement, accreditation, licensing, and operational activities.
- Required by Law: When disclosure is required by federal, state, or local law.
- Public Health and Safety: For approved public health activities or to prevent serious threats to health or safety.
- Law Enforcement or Court Orders: When legally authorized or required.
Your Rights Regarding Your Health Information
You have the right to:
- inspect and obtain a copy of your records,
- request electronic copies of your records,
- request corrections or amendments,
- request confidential communications,
- request restrictions on certain uses or disclosures,
- receive an accounting of certain disclosures,
- receive a paper copy of this Notice upon request,
- and file a complaint without fear of retaliation.
Breach Notification: If a breach occurs that may compromise the privacy or security of your information, we will notify you without unreasonable delay and as required by law.
